Appointment Policy
Our office will contact you at least 24 hours in advance to remind you of your scheduled appointment. Please bring a list of your current medications or the medications themselves and the names of your physicians with you to the appointment. If you are unable to make your appointment, we encourage you to cancel at least 24 hours in advance. If you fail to cancel or show for an appointment, you will be charged $25.00. We ask for the consideration to cancel appointments so we can open the appointment up for someone needing attention due to an acute illness. |
Medication Refills
When you need to refill a prescription, call the pharmacy where the prescription was last filled at least 24 hours in advance of needing the medication. We respond to pharmacy requests received prior to 3:00 pm by the end of the business day. Requests received after 3:00 pm will be addressed the following business day. If you need a refill at the time of your office visit, please inform the nurse while you are in the treatment room. Medication refills will not be called during evening or weekends hours. If you receive a change in your prescriptions during an office visit, written prescriptions for previous medications are considered void. For patients taking Predisone, you should not stop the medication abruptly. Therefore, it is important for you to call your pharmacy early for a refill so you do not run out of the medication. |
Billing
Our office will file your insurance claim with your primary insurance company and if applicable, secondary insurances. It is your responsibility to provide us with current insurance information so claims will be paid in a timely manner. You are responsible for claims not reimbursed by your insurance company, and a monthly statement will be sent if there is any balance due. Co pays, deductibles and coinsurance will be due at the time services are provided. We will verify your insurance coverage when you are scheduled for specialty procedures in our office and we will notify you of associated coinsurances and deductibles. Patient Responsibilities
It is important for you to be honest with us regarding your health, family history and lifestyle so we may provide you with the highest level of medical care. We will provide you with information on how your disease may be affected by your lifestyle decisions. To receive the greatest benefits from our medical care it is important to follow our instructions regarding medications, diagnostic testing, referrals, activity level and follow up.

Confidentiality
Our practice treats all aspects of your care confidential. You will be asked to review and acknowledge our confidentiality policy. You may also be asked to sign a release of medical information to allow our office to communicate with others involved in your medical care. |
